To apply for services at Seattle Counseling Service, download the application below. Mail or Fax us the completed application or just bring it by our office. Our Client Services Coordinator will then contact you to answer any questions you may have and schedule your first appointment.

Agency Information

• Hours/Days: Business office: Monday thru Thursday open 8:00 am to 7:00 pm. (some sessions run later).
  Friday 8:00 am to 5:00 pm
• Funding: Non-profit 501(c)3; King County Mental Health Board, United Way, Title XIX, client fees, donations, third party reimbursement. Licensed by Washington State; King County mental health contract, Ryan White Foundation, Domestic Violence Contract (same sex only), Sexual Assault Grant, Crime Victim's Compensation Program, Private Grants.
• Languages other than English: Spanish; other language interpreters including ASL are available.
